Our MyChart patient portal allows you to easily renew prescriptions, view lab results, and message your provider at any time. We also offer after-hours, same-day, next-day, and video services to avoid trips to the emergency room or urgent care.

    • The phone line is open from 8:00 am to 5:00 pm Monday through Friday

    • If you are unable to keep your appointment, please notify the office as soon as possible. Failure to notify us may result in a charge (see no-show policy below)

    • Please arrive 15 minutes early, to give us time to make certain your records contain accurate and complete information

    • Please bring all your medications with you, a picture ID, and your insurance card

    • Emergencies must be evaluated promptly, by calling 911 or going to the nearest Emergency Room

    • An on-call provider is available by telephone for non-emergent issues. Please call (616) 957-9237 after-hours to be directed to the provider on-call

    • Patients must be established with our office to use the on-call service

  • We kindly request that you notify us at least 24 hours in advance if you need to cancel or reschedule your appointment. Failure to do so will result in a charge of $25, which is not covered by your insurance.

    We understand that emergencies and unexpected events may arise. Repeated no-shows or last-minute cancellations prevent us from seeing other patients who may have needed our services. By giving us ample notice, you allow us to offer your time slot to another patient.

    • Patients should submit requests for refills with a minimum of 2 days BEFORE the medication is going to run out to give their provider a sufficient amount of time to respond to the request

    • Patients can make requests for prescription refills prior to 4 PM Monday through Friday

    • Requests called in after 4 PM will be received the following business day

    • Our automated phone system will guide you as to how to leave a message for your refill, or you can submit a request on the patient portal

    • Refill for controlled substances require a signed Controlled Substance Agreement and a urine drug screening

  • Full payment, including any co-payment that your insurance may require, is expected at the time of service. If you are experiencing financial difficulties and need to make special payment arrangements, please ask to speak with a member of our billing department. We are here to help you.

    • Always bring your current health insurance card to your appointments

    • Please notify us at time of check-in of any changes in insurance, address, telephone, or family status

    • Please pay your co-pay or deductible balance and co-insurance amount at the time of service

    • You will be expected to pay in full (self-pay) if: you do not have insurance, we are out-of-network with your insurance plan, we are unable to verify your insurance coverage, or you are unable to present a valid member identification card from your insurance carrier at the time of service

    • Referrals will be processed within 24 hours of your visit by our referral team

    • Most specialists prefer to contact you directly to schedule the appointment within 2 weeks, or 24 hours if it's urgent.  Some offices can take longer than 2 weeks

    • If you don’t receive a call from the specialists with a date within 2 weeks, please call our office at 616-957-9237 and dial extension 8019 to be connected to our referral coordinator

    • Please know that some specialists are booked out several months in advance.  We recommend you to ask the specialist about being placed on a cancellation list if available
